Referrals
For referring doctors
One page for your staff: how to send a patient, what we send back and how to reach us.
You send us a patient with diabetes, glaucoma, a family history or an eye complaint. We examine them, treat what is ours to treat, and write to you.
How to refer
- 01
Send the referral
Use the referral form on our patient portal, or call. Say what you would like us to look at.
- 02
We schedule the visit
The patient hears from our office, and we tell you if we can't reach them.
- 03
We write back
A summary after the visit, and a call to you if we send the patient on to a surgeon.
What we send back
After a routine or medical visit
A one-page summary with our findings, what we did and when we will see the patient again.
When a condition changes
A call or a note the same week, so your plan can change too.
When we send a patient on
We call the surgeon ahead of the visit and send the records with the patient.
What we ask of you
Who you are sending
Name and phone number. We collect health history at the visit, not on a website form.
Why
In your words, on the referral. We do not ask patients to describe it on this website.
How to reach you
A direct line for questions about the patient, and the fax or portal you prefer.
Questions from referring offices
Do you treat the patient or only examine them?
Both. We treat what optometry treats in Pennsylvania, and we send the patient to a surgeon when that is the next step.
Do you take medical insurance referrals?
Yes, a medical visit is billed to the patient's medical insurance. The office checks benefits before the visit.
Can my patient book without calling?
Yes. The booking link is on every page, and it is also open to patients you have not sent.
- Sudden vision loss, flashes of light, a curtain over your vision, a chemical splash or an eye injury needs care now. Call our office right away or go to an emergency room. If you can't reach us, call 911.
